Terms
The terms of using this website.
About these terms
This website is run by Spine Treatment Centers, the trading name of Southwestern Ohio Disability Associates, LLC, 4050 Executive Park Drive, Suite 110, Cincinnati, Ohio 45241.
These terms apply whenever you use the site. By browsing it or sending us a booking request, you accept them. If you do not accept them, please do not use the site — call us on (513) 241-4230 instead and we will help you the ordinary way.
Our Privacy Policy explains what we do with the information you give us, and forms part of these terms.
This site is information, not medical advice
Everything on this website is general information about the conditions we treat and how our practice works. It is not a diagnosis, not a treatment plan, and not medical advice for your situation.
Nothing here should be used to decide whether to seek care, to delay seeing someone, or to stop a treatment you are already having. Only a clinician who has examined you can advise you on your own condition.
Using this site does not make you our patient
Sending a booking request starts a conversation. It does not create a doctor–patient relationship, and it is not a confirmed appointment.
That relationship begins when you have been seen by one of our providers and we have accepted you into care. Until then, we are not treating you and we are not monitoring anything you have told us for anything urgent.
We also cannot guarantee that a request will lead to an appointment. We will always call you back and talk it through.
In an emergency
This website is not for emergencies, and nobody is watching it in real time. A booking request may not be read for hours.
If you are having a medical emergency, call 911 or go to your nearest emergency room.
Signs that need emergency care rather than a booking request include chest pain, difficulty breathing, sudden weakness or numbness, loss of bladder or bowel control alongside back pain, or pain following a serious accident.
If you are in crisis or thinking about harming yourself, call or text 988.
Who can use this site
Anyone can read it.
If the patient is under 18, a parent or guardian should complete the booking request and will be the person we contact. We treat patients under 18 and the form is open to them — we only ask that an adult sends it.
If you live outside Ohio, you are still welcome to get in touch. Most of our care is for people living in Ohio, but we will talk through what is possible.
Please give us accurate information. We rely on it to call you back and to check what your cover pays for, and we may not be able to help if it is wrong.
Using the site properly
Please do not:
- Send a booking request for someone else without their knowledge, unless you are their parent or guardian.
- Submit false information, or someone else’s details as your own.
- Try to get into parts of the site, or anyone’s information, that are not yours.
- Interfere with the site or the systems behind it, or use anything automated to collect from it.
- Use the site for anything unlawful.
Our content
The text, photographs, logo and design on this site belong to Spine Treatment Centers or are used with permission. You are welcome to read them, print a page for your own use, and share a link.
Please do not republish our content, use our name or logo to suggest we endorse something, or present our material as your own.
Patient photographs and stories appear with permission. Please do not copy or reuse them.
Links to other websites
We sometimes link to other organisations — insurers, the Ohio Bureau of Workers’ Compensation, health information. We do not control those sites and are not responsible for what they contain or what they do with your information. Their terms and privacy policies apply once you leave ours.
Availability
We try to keep the site up and accurate, but we cannot promise it will always be available or always current. Hours, services, the providers listed and the insurance we accept can change, and the site may be behind.
If something matters to your decision — whether we take your plan, whether we treat your condition — call and check rather than relying on the page.
We may change, suspend or withdraw any part of the site at any time.
Limits of our responsibility
This section is about the website. It does not limit our responsibility for the care we give you — that is governed by law and by our professional obligations, and nothing here changes it.
For the website: we provide it as it is. To the extent the law allows, we are not responsible for loss caused by the site being unavailable, by information on it being out of date, by a booking request not reaching us, or by anything on a site we link to.
We do not exclude, and cannot exclude, responsibility for:
- death or personal injury caused by our negligence;
- fraud or fraudulent misrepresentation;
- anything else the law does not permit us to exclude.
Governing law, changes, and contact
These terms are governed by the law of the State of Ohio, and the courts of Ohio have jurisdiction over any dispute about them.
If part of these terms turns out to be unenforceable, the rest still applies.
We may update these terms. The version on this page is the one that applies, and it carries the date it was last changed.
